Patient Profile:Individuals aged 18 or older with opioid use disorder, alcohol addiction, mental health issues, or seeking smoking cessation, who can provide valid OHIP and photo ID.
Patient Instructions
  1. 1Set up an intake appointment by contacting us by telephone or on-line.
  2. 2Visit a RTR clinic and present your OHIP card.
  3. 3Be 18 years of age or older.
  4. 4Have valid photo identification, including a valid OHIP number.
  5. 5Opioid use disorder evidence must be present as per DSM-V criteria.
  6. 6Agree to follow program policies & procedures.
  7. 7Give full consent prior to starting medication assisted treatment (to be agreed upon and signed by the patient).
  8. 8A urine sample will be obtained periodically to assess for recent drug use.
