Patient Instructions
- 1Please do not wear scented products to the medical clinic.
- 2Phone lines are exceptionally busy between 9-11AM.
- 3For all medical emergencies, call 9-1-1.
- 4Bring your BC Health Card, eye glasses (if needed for driving), and any relevant medical information for Drivers Medical Exam appointments.
- 5Allow at least 6 weeks for medical chart transfers.
- 6Payment for chart transfers/copies can be made by cheque, credit card, or debit only.
- 7For ADHD forms, fill out as directed and drop off at the clinic for review.
- 8If scheduled for a Meet & Greet, print and fill out the intake form and drop it off at the clinic prior to your appointment.
