Patient Profile:Children and youths with developmental disorders can benefit from services like Behaviour Consultation, Psychological Assessments, and Speech and Language Therapy, among others.
Patient Instructions
  1. 1Present a VALID ONTARIO HEALTH CARD at every visit.
  2. 2Fully complete intake questionnaires and submit online or return prior to first visit.
  3. 3Allow 1.5-2 hours for the first appointment.
  4. 4Arrive 15 minutes prior to appointment for registration.
  5. 5Bring any prior assessment reports, school report cards, Individual Education Plan, or other relevant documentation.
  6. 6If there are any child custody arrangements, relevant documentation (ie. court orders) is required prior to accessing services.
  7. 7Complete the Parental Agreement Form and return at first visit.
  8. 8Patients on medication require close monitoring and follow-up and therefore are responsible for keeping their scheduled appointments within their prescription allowance time.
  9. 9Provide the clinic notice of a need for a prescription renewal 2 weeks prior to running out of medication.
