Patient Instructions
  1. 1Fill out ALL blanks in the intake package. If you do not know the answer to a question, please take the time to look up the correct information (Phone #’s, Fax #’s, addresses and Doctor information is very important!).
  2. 2If any questions don’t apply to you or your symptoms, just leave them blank.
  3. 3If you do not have a person to sign as a witness, the clinic will sign those pages for you at your appointment.
  4. 4Bring your glasses to your appointment, as additional paperwork may need to be filled out.
  5. 5Bring your calendar to book future appointments.
  6. 6If ANY imaging (X-RAY, ULTRASOUND, CT and/or MRI) has been taken in relation to the area of injury, please have a copy of the report sent to the clinic before your appointment (FAX# 519-235-2730., E-MAIL: russettrehab.@gmail.com).
  7. 7Vertigo clients need to bring a drive to appointments.
  8. 8If treating the upper extremity please wear a tank top and for the lower extremity please wear shorts so the affected area may be assessed and treated appropriately.
  9. 9Cancellations received without 24 hours notice will be charged 50% of the service fee. Failure to provide any notice will result in the full service fee being charged.
