  2. 2If this is your first visit, please select the “Initial Visit” appointment online. You will be sent a link to set up your profile and fill out online paperwork.
  3. 3If you are booking a return visit for regular footcare, wart treatment or orthotics and have been to the clinic in the last 3 years, please book accordingly.
  4. 4If you have not been to see us in 4 years or more, please book a Re-assessment.
  5. 5Contact your insurance company for coverage prior to your visit.
  6. 6Bring completed forms to your appointment.
  7. 7Review office policies regarding late or missed appointments, as a $25.00 fee will be billed.
