Instructions
- If your insurance requires a referral, have your Primary Care Physician fill out a consult request form and fax it along with a completed medication list, pertinent office notes, EMG’s, radiology, MRI/CT reports, and a copy of your health insurance card to 207-942-5631.
- If your insurance does not require a referral, contact the secretary of the provider you wish to see. The office will need to obtain any pertinent office notes, EMG’s, radiology, MRI/CT reports for the physician to review. You can have them faxed to 207-942-5631 or mailed to 78 Ridgewood Drive Bangor, ME 04401.
- If you do not wish to see a specific physician, contact the referral department at 207-307-8948.
