Patient Instructions
  1. 1A valid health card is required or fees will apply (CASH ONLY)
  2. 2Medical masks are recommended at both locations
  3. 3Completion of forms is at the discretion of the attending physician
  4. 4Online registration is available for same day in-person visits
  5. 5Check-in with the receptionist upon arrival
  6. 6If you do not have a valid health card, cash payment is required, and a valid health card must be provided within 15 business days for a refund
  7. 7For Tele-Clinic: have health card ready, take temperature, take blood pressure (if applicable), have a list of allergies, child's weight (if applicable), list of current medications, be available to receive a call back (phone number may appear as “blocked” or “no caller ID”), and be in the province of Ontario when you receive the call
