The Center for Health and Wellbeing at the University of Vermont offers medical, mental health, health promotion, and wellbeing services to full-time UVM students. Services include general medical care, counseling, psychiatry, and integrative wellness coaching.

Instructions

  1. Submit Required Forms: Have your allergist complete the Allergy Clinic Info Checklist and fax it to us: 802-656-8178 or 802-656-8001. We must receive this form before you arrive on campus.
  2. SHS Approval Required: Once your documentation is reviewed, we’ll notify you via the MyWellbeing portal if you’re approved for allergy injections or if additional information is needed.
  3. Send or Bring Your Serum and Paperwork: Store it as instructed by your allergist and drop it off on move-in day or when you arrive on campus. Send serum paperwork via secure file transfer to [email protected]. Fax serum paperwork? 802-656-8178. Mailing it?
