  2. 2Please limit your visit to 1-2 of your most concerning issues, and book follow-up visits for any remaining concerns.
  3. 3Walk-in visits are to be limited to one problem only.
  4. 4Do not discuss test results over the telephone. Book an appointment to review test results.
  5. 5Ensure your contact information is up to date with the reception.
  6. 6Prescription refills are not offered by telephone. Book an appointment or come as Walk-In if medication is running low.
  7. 7Prescriptions for narcotic and controlled medication cannot be given for longer than 1 month to regular patients.
  8. 8No narcotic or controlled medications are prescribed to Walk-In patients.
  9. 9Do not refill prescriptions faxed by different pharmacies.
  10. 10Telephonic consults are not offered by physicians. Book an appointment to speak to a physician.
  11. 11Provide written, witnessed consent form to release medical information to anyone.
  12. 12Bring Alberta Health Card and 1 Piece of Govt. Photo I.D. (e.g., Driver License) at the time of first visit.
  13. 13For Marine Medical Examination, bring two valid photo IDs, Alberta Health card, and CDN number.
