Bring at least one of the following documents to your Health Center visit if you don’t have health insurance or if your household income has recently changed: Most recently filed income tax return, Paychecks stubs from current or most recent job, Most recent W-2. If unable to bring documents, a one-year self-attestation form can be filled out. Forgetting documents for more than three appointments will result in full price charges.
