  1. 1PLAN YOUR APPOINTMENTS IN ADVANCE to avoid running out of medication or delaying follow up.
  2. 2Please come wearing a disposable three layered medical mask IF you have any flu or cold symptoms. If you do not have any symptoms, masks are optional.
  3. 3Please use the hand sanitizer in the waiting room upon arrival and follow the signs.
  4. 4If you have an in-office appointment booked and are ill the day of, please let us know and we can reschedule or adjust to a telehealth visit as needed.
  5. 5Only the patient (and one parent/caregiver for children and those who need assistance) is allowed in the doctor's office.
  6. 6For telehealth visits, be ready near your phone at the time of your appointment and the physician will call. Ensure you are in a quiet, private location.
  7. 7If anticipating a prescription in your visit, have the FAX number of the pharmacy of your choice.
  8. 8For forms, please email them to ghfm@goodhealthmd.ca in PDF format. Ensure patient portion is filled out and signed.
