  2. 2If a referral has not been sent by a physician or nurse practitioner, do not contact our offices to request an appointment.
  3. 3For telephone visits, wait by your telephone at the scheduled date and time.
  4. 4For telephone visits, be prepared to wait up to 45 minutes for your call.
  5. 5For telephone visits, have your health card, list of medications, and any other relevant information available.
  6. 6For telephone visits, have the name of your pharmacy and their telephone and fax numbers available if a prescription is required.
  7. 7For telephone visits, if not comfortable speaking English, have a family member or trusted friend by the telephone to help with translation.
  8. 8For telephone visits, answer calls with an “unknown” or “blocked” Caller ID, or from an unexpected number.
