Instructions

  1. We request 24-hours notice if you need to cancel your appointment.
  2. Patients needing daycare, physical, and/or any type of forms to be completed by the physician must allow the physician a minimum of 7 business days at least for completion of the forms.
  3. Patients requesting medical records must sign a release of medical records and allow 10 business days for medical records to be processed. There is a medical records fee of $25.00.
