Patient Instructions
  1. 1Proof of vaccination is not required but is recommended for all visitors and Designated Care Partners (DCP).
  2. 2Two visitors per patient at one time, visitors may alternate.
  3. 3No children under 12 years of age are allowed. Children 13 to 16 years of age must be accompanied by one adult.
  4. 4Wash your hands before and after visiting. Please use the hand sanitizer solution provided in patient rooms and main entrances.
  5. 5Bring your health card to every appointment and give it to reception when you arrive.
  6. 6Notify reception if you have changed your address or telephone number.
  7. 7Check the expiry date on your health card and arrange for a new one before it expires.
  8. 8Notify the OHIP office if your card is lost, stolen, or expired.
  9. 9Contact your pharmacy to request prescription refills.
  10. 10If you cannot go to a diagnostic test appointment, you must call the diagnostic lab to change your appointment.
  11. 11If you cannot go to a specialist appointment, please contact your specialist's office to reschedule.
