Instructions

  1. If your infant was born off-post, please call the clinic to schedule the first appointment and coordinate to send a copy of baby's discharge note.
  2. Please call to re-schedule the appointment if you or your child is ill.
  3. For 2- to 17-year-old well physicals and sports physicals, if your child has been seen in the last year, paperwork can be completed without an appointment via the MHS GENESIS Patient Portal; complete the parent portion first.
