Instructions

  1. Inform a member of the patient’s healthcare team if they would like to have an individual visit with one of the Chaplains.
  2. Children 13 years and under must have a signed consent form from a parent or guardian in order to allow one-on-one chaplain visits.
  3. Notify any staff member if you would like an interpreter.
