Patients should bring their Advance Directive, Copay or deductible, Driver’s license, Insurance Card, Medicare/Medicaid card, Name and Social Security Number of the insured, Referral forms from their physician (if required by their insurance carrier), Referral/Order Guidelines, and Worker’s compensation information (if applicable) when registering for an appointment or hospital stay.
  2. Patients can sign into MyChart, the online patient portal, to view records, request medication refills, access test results, and manage appointments.
  3. Screening with a metal detector wand is required for entrance into the Emergency Department.
