  1. Bring your insurance cards if you have them plus your last two pay stubs or documentation of income.
  2. Bring proof of residency and documentation of your HIV status to see if you qualify for additional services.
  3. If you are late by more than 10 minutes for an appointment, you may need to reschedule. Please call Thrive Alabama at (256) 536-4700 if you will be late.
