Instructions

  1. Review Logan Health’s notice of privacy practices, patient consent and financial agreement, advance directives, patient rights and responsibilities, organizational code of ethics, messages from Medicare and Tricare, and truth in lending.
  2. Fill out and sign an authorization form, show photo ID and pay the charge for copying to get copies of medical records.
  3. For personal copies of X-rays and other imaging on a CD, there is a $35 charge.
