About the Clinic

Community Action Corporation of South Texas (CACOST) is a non-profit organization providing affordable and accessible medical, dental, behavioral, and pharmaceutical health care services through nine Federally Qualified Health Center facilities. They also offer senior nutrition, utility, weatherization, and early childhood intervention programs across a 16-county service area.

Instructions

  1. Download the enrollment application and email your completed application with the required documents to [email protected] or drop it off at one of our health center locations.
  2. You may also pick up a copy of the application at any of our health center locations if you are unable to download it.
  3. Bring Driver's License/ID, Birth Certificates, School Records, etc. for all minors in the household AND Your TX Benefits Award Letter OR Last Two Pay Stubs OR Employment Verification Form OR Social Security/ SSI/ Disability Award Letter OR Child Support Documentation, Proof of Residency to establish eligibility for healthcare assistance.
