  2. If a referral is required, request it from your PCP and allow at least three days for them to forward it before calling our office.

  1. Park in the DeYoung Pavilion Surface Parking or Underground Parking.
  2. Drop passengers at the entrance of the Pavilion.
  3. From Surface Parking, enter the DeYoung Pavilion from the Surface Lot, take the elevator to the 4th floor.
