Instructions

  1. Please call the office for instructions and to set up a telemedicine or phone appointment during normal office hours.
  2. Take your time to answer the new patient questionnaire and bring it with you at your first visit.
  3. Fill and sign the Release of Health Care Form if you would like us to have access to medical records from your other physician(s). You could mail or fax it directly to your other physicians to request a copy of your medical records ahead of your visit or bring it with you.

Getting Here

  1. Orion Medical Center is located just north of Dunham's Sports and Dollar Tree.
  2. We are located on the lower level with the separate parking space.
