Instructions

  1. Referrals are required to access services UNLESS you are an "unattached" patient (do not have a Family Doctor).
  2. If you have a Family Physician, Midwife or Nurse practitioner, have your Provider fax the Referral to 604-409-3979.
  3. Referrals are not required for emergency IUDs.
