  2. Schedule a preadmission appointment for required tests and to meet with a nurse for health history, surgical consent and pre/post-op education.
  3. Bring driver's license or photo ID, social security card or number, insurance cards, physician orders for surgery, list of current medications and allergies, and copies of advanced directives or power of attorney to your preadmission appointment.
