Instructions

  1. You must be referred to our office by your family physician, walk-in clinic physician or another specialist.
  2. You must present a valid Ontario Health Card or your up-to-date Interim Federal Health paperwork at the time of your appointment.
  3. If you have seen one of our surgeons in the past, but now wish to be seen for a new or different problem, you must see your family doctor first.

Getting Here

  1. Located on the east side of Dunwin Drive, just north of Dundas Street, directly behind 2145 Dunwin Drive.
  2. Enter through the main door to the right of unit #7 (marked #27/28).
  3. The patient entrance is on the main floor on the left-hand side of the vestibule.
