  1. Please notify us at least 24 hours in advance if you need to reschedule or cancel your diagnostic testing appointment. Patients who do not properly prepare for or who do not provide adequate notice will be subject to a cancellation fee of $50.

  1. Providence Office: Located on the south-side of the USA Health Providence medical office campus in Building D on the third floor.
  2. Mobile Infirmary Office: Located on the campus of Mobile Infirmary Medical Center on Springhill Avenue between Louiselle Street and Mobile Infirmary Drive.
  3. Springhill Office: Office is located in the lobby of the Heart Center/Medical Office Building One.
