  1. Print and fill out Patient Health History, HIPPA Acknowledgement, Patient Registration Form, and Notice of Privacy Practices before your appointment to reduce wait time.
  2. For Glucola Test: DO NOT HAVE sugar, oatmeal, breakfast bars, corn grits, regular soda, jelly, candy or gum, fruit juices, fruit (canned or raw), milk, sweet tea, honey, sugared cereals, syrup, carrots before the test. YOU MAY HAVE sausage, ham, salads (chef, green), tuna salad, water, eggs, sugarfree candy or gum. Focus on high proteins and no carbs.
  3. Avoid medications if possible, especially in the first three months of pregnancy. YOU MAY USE: Tylenol or Extra Strength Tylenol (up to 4 grams daily), Mucinex, Robitussin, Chloraseptic throat spray or lozenges, Saline nasal spray, Metamucil, Milk of Magnesia, Senkot S tablets or Surfak & Colace, Tums & Rolaids, Riopan, Mylanta, Maalox, Prilosec, Nexium and Pepcid AC, Kaopectate or Immodium, Preparation H, Tucks, Anusol and Anusol HC, 3 Day Monistat vaginal cream, Benadryl, Tylenol PM, Unisom/Vitamin B6. DO NOT USE: Ibuprofen, Motrin, Aleve, Advil, Aspirin (unless directed), Alcohol, Cigarettes, Caffeine (in moderation only), Tetracycline, Sulfa, Afrin nasal spray, Accutane, Tranquilizers and sedatives, Herbal/Natural Compounds (without doctor’s permission), Pepto Bismol.
