  1. Program referrals are made by Home and Community Care Support Services Champlain (HCCSS), as well as health care professionals and other Community Support Services and programs. Inquiries are always welcome by calling the program directly.
  2. For information on eligibility, contact Home and Community Care Support Services at 310-2222 (no area code required).
  3. A family member or designate must be there to meet the client at the end of the day for transportation.
