Instructions

  1. All patients with out of pocket costs will be required to leave a credit card on file to cover copays, coinsurance, and deductibles.
  2. First time visitors: Please arrive 15 minutes prior to your appointment time to allow time for registration.
  3. Bring insurance card (REQUIRED), Photo ID (REQUIRED), a list (or the bottles) of all your medications (oral and topical), a list of your medical history and physicians, and printed forms if you did not submit online.
