The Chickasaw Nation Division of Health provides comprehensive healthcare services including audiology, behavioral health, dental, imaging, medical family therapy, optometry, pediatrics, pharmacy, physical therapy, and primary care. Services are available to Chickasaw citizens and First Americans at various health clinics and the Chickasaw Nation Medical Center.

Chickasaw citizens and First Americans of all ages, including children and pregnant women, who are patients of the Chickasaw Nation Department of Health.

Instructions

  1. Patients must have an appointment to enter any Chickasaw Nation Department of Health (CNDH) facility.
  2. COVID-19 testing is required for most dental procedures and preventative care treatments, to be completed within 72 hours of a scheduled appointment.
  3. Appointments will be rescheduled if a patient tests positive for COVID-19.
