  1. For immigration physical exams, please type in your info, print, and bring the completed I-693 form to your visit, using black ink. You will need to fill out only page 1 and the first line of pages 2-13. Otherwise, you will need to fill out the I-693 in our office.
  2. For aviation medical exams, log into the FAA site MedXPress and complete the form. Please let us know your confirmation number before your visit so we can prepare.
  3. Walking in is welcome, but may incur a wait time.
