Instructions

  1. Prior to your appointment, you will receive an email and/or a text from Phreesia. The included link allows you to securely input and correct all of your personal information, including uploading your driver's license and insurance cards. You also have the option of paying your co-pay and/or any outstanding balances.
  2. For physicals and wellness exams, patients will be charged a fee if they don't show, or if they cancel within 24 hours of the appointment.
  3. When calling for a prescription refill, please allow 48 hours for the request to be completed.

Getting Here

  1. One of the Rochester Hills locations is in the Wellpointe Building, located at 1701 South Blvd. E., west of Dequindre and east of the Beaumont building.
  2. PRISM Medical Group is comprised of 5 suites in this building, 3 on the second floor and 2 on the first floor.
  3. The second Rochester Hills location is in the Executive Place Building, located at 1055 South Blvd. E., on the north east corner of John R and South Blvd. The Bloomfield Hills location is at 50 Big Beaver Rd., just east of Woodward, on the first floor.
