Instructions

  1. If your insurance is an HMO/managed care plan and you are required to obtain a referral from your primary care physician, the referral must be obtained before you can be seen for an office visit.
  2. If you need physical, occupational, or hand therapy, you will need a referral from your physician or medical provider for insurance to pay for your rehab treatment.
  3. Prescription refills require (3) business days (72 hours) to process. No refills are issued during the evenings, weekends, or holidays.

  1. Our main office building is located on the east side of Cattlemen Road, ½ mile north of Bee Ridge Road (Exit 207 off I-75).
  2. It is a freestanding building with our name across the top.
