Instructions

  1. Active duty service members should report to the Cardinal/Swan Team during sick call hours (7:30-8 a.m.) for same-day appointments; appointments are first come, first served and cannot be made over the phone.
  2. All service members will sign their name in the SICK CALL Record book to verify date, time, and presence.
  3. Medication renewal requests are preferred through the MHS GENESIS Patient Portal, including medication name(s) in the message.

  1. From the Madigan Army Medical Center Gate (Interstate 5 Exit 122) continue on Jackson Ave. to the second traffic light (on base) and turn right at Beaumont Ave. Madigan Family Medicine is located on the ground floor of the Medical Mall.
