Instructions

  1. If your infant was born off-post, please call the clinic to schedule the first appointment at 253-968-3066, Option 3 and coordinate to send a copy of baby's discharge note.
  2. If your child is also due immunizations, they will be done during the Well Visit. Please call to re-schedule the appointment if you or your child is ill.
  3. If your child has been seen in the last year, we will complete your child’s paperwork without an appointment. You may send in paperwork via the MHS GENESIS Patient Portal. Please make sure to complete the parent portion first.
