Instructions

  1. New Patients: Please arrive at least 15 minutes before your scheduled time to complete the registration process. Bring your driver’s license/government ID (or other photo ID) and insurance cards. If you don’t have insurance, bring proof of household income: tax forms, paycheck stubs, bank statements, or MDHHS eligibility forms.
  2. Returning Patients: Please arrive at least 15 minutes before your appointment time to sign in and do any updates to your information that may be needed. Bring your driver’s license/government ID and insurance cards.
  3. Late Arrival: If you don’t arrive on time, you may be considered a walk-in patient and need to wait for the next available provider or be rescheduled.
