  1. Complete online forms with as much information as possible before the first appointment.
  2. Older children and youth should complete their section of the forms.
  3. Email, fax, mail, or bring all relevant medical and mental health reports to the first appointment if requested (school reports, individual education plans, past psychology assessments, other relevant medical documents).
