About the Clinic

Signature Health is a non-profit Federally Qualified Health Center in Northeast Ohio, providing integrated mental health, addiction recovery, primary care, dental, infectious disease, and pharmacy services to patients of all ages.

Signature Health serves individuals of all ages, including Medicaid and Medicare patients, with a sliding fee scale available for eligible uninsured individuals.

Instructions

  1. For fastest access to a new patient visit, drop-in online or walk-in at one of our health centers.
  2. If you are regularly unable to make it to your appointments because of lack of transportation, please let us know.
  3. If this is a life-threatening emergency, please go to your nearest emergency room or dial 911.
