Instructions

  1. 24 hours notice is required to cancel or rebook any appointments, or fee will apply.
  2. Please do not upload your requisition from your physician.
  3. If you miss your appointment there is a charge of $25, if you miss more than one following your first missed appointment the fee is increased to $50.
