  2. Bring Photo ID, Insurance card, Current medication list, and Previous medical records (if available) to your appointment.
  3. If taking a “blood thinner” medication, notify the Pain Treatment Center ahead of your visit.

  1. Turn at the Emergency entrance sign and park in the small lot on your left, across from the ER entrance.
