- For couples or patients in a relationship planning to parent together or provide gametes, all individuals will require referral and assessment prior to commencing treatment.
- When completing the consent for medical information release, both partners MUST sign if applicable, and the consent MUST be witnessed by a separate individual.
- Patients who would like to have a language interpreter or support person present are welcome to make these arrangements ahead of time. If registered as a couple, both partners must provide written consent for a support person to be present.
