Instructions

  1. For prenatal appointments, complete the New Patient Questionnaire online or arrive 20 minutes early to fill out the form in the office.
  2. Bring your AB Health Care Card to appointments and ensure it is accurate.
  3. If new to Alberta and eligible for AB Health Care, apply ASAP to ensure newborn insurance at delivery.

  1. Located in the Rockyview Health Centre Building II at 1016 68th Avenue SW, on the second floor (unit 200).
  2. The easiest way to access the clinic is to enter the building on the south side, where elevators are visible.
  3. If entering from the north side, follow signs to the elevators.
