- Bring a list of all medications you currently take, preferably on a clinical summary from your primary care physician.

- The clinic is located in the Northwest Professional Center, in the first office building on Tremont Road northeast of the "Five Points" intersection of Tremont Rd, Fishinger Rd, and Northwest Blvd, just behind the PNC Bank.
