Instructions

  1. Call 972-566-7576 to start the new patient process and paperwork.
  2. For school forms, allow a seven-day turnaround time and expect a $25.00 charge. Yearly visits must be scheduled prior to forms being completed.
  3. If you have a fever or cough/shortness of breath worse than usual symptoms, you will be asked to reschedule.
