Bring Passport, INS Form I-693, Supplemental Form To I-693 Adjustment of Status Applicant’s Documentation of Immunization, Current Medications, Immunization Records, and any prior blood test results for AIDS (HIV titers), syphilis (RPR test), or measles (rubeola), rubella (German measles), mumps, or varicella (chickenpox) for immigration medical exams.
  3. All expenses must be paid at the time of your exam.
