Instructions

  1. Your Name, Date of Birth, and Health Card are required to book an appointment.
  2. For prescription refills, leave a voicemail with pharmacy name/number and prescription names (may take up to 5 business days), send an email to [email protected] with subject 'PRESCRIPTION REFILL', ask your pharmacy to send an e-request or fax, or visit the same-day clinic.
  3. For forms that need to be filled, physicians have up to 30 days to complete them. Payment options include cash or card, and a receipt will be provided upon payment. Some fees may be eligible for reimbursement by specific carriers; consult your insurance provider.

Getting Here

  1. To access the parking garage, head towards the entrance of the Radisson hotel, follow the ramp up, take a ticket at the parking meter, and proceed left towards the P3 and P4 Entrance doors.
