  1. Print and complete forms prior to appointment (Patient Registration Form, Financial Policy, Review of Systems, Medical Records Request, Medication List, Personal / Family History, Patient Rights, HIPAA Notice of Privacy Practices, Patient Record of Disclosures).
  2. Patients having procedures at Northeast Endoscopy Center should download, print, read, complete, and bring the signed Patient Rights form to the procedure, dated prior to the procedure date.
  3. If insurance plan is HMO or POS, notify Primary Care Physician for referral processing.

  1. Lawrenceville Office: Northeast Endoscopy Center is located in the same building in suite 110 on the first floor.
  2. From Atlanta, take 85 N to 316 E, exit 316 on Hwy 120 (Duluth Hwy) and turn right.
  3. At Chick-Fil-A, turn right onto Professional Dr, then next left on Wellness Way.
