Instructions

  1. Patients applying for the Discount Program must show proof of income or provide documentation as required by First Choice Primary Care.
  2. Bring Photo ID, 2 forms of Proof of Address (Utility Bills, Lease agreement, Letter from homeless shelter, or other mail showing current address), and Proof of Income (Paycheck stubs showing one month’s worth of income, last year’s tax return for self-employed, letter from employer for cash pay, Child Support Income, Social Security or Disability Award Letter, or official document for no income).

  1. FCPC East is located inside the Health Department at 171 Emery Hwy.
  2. Bruce Elementary and LH Williams Elementary are School-Based Health Clinics.
