  1. If you are visiting Manchester Memorial Hospital for a test or procedure, you are required to pre-register. Call our pre-registration line at 860.872.5150.
  2. For insurance reimbursement, a physician referral is needed to utilize our services. The referral can either be faxed to us or we can help you over the phone. Fax: Please send us your most recent doctor's progress note, pertinent labs, H&P, and other supporting documentation of diagnosis to 860.474.1700. Be sure to have your provider include the nutrition and diabetes related ICD-10 diagnostic code related to the reason for referral. If your provider is unsure, they can always use the preventative code Z71.3 Dietary Surveillance and Counseling. Phone: You can call us at 860.647.6824 and provide the necessary information over the phone or you can have your doctor contact us directly. For insurance reimbursement, it is helpful for our office to have your doctors most recent progress notes and any related nutrition diagnosis, this will help with reimbursement. Please fax the notes to us at 860.533.3417 or simply provide us with this information over the phone.
  3. If you receive pain medication, please arrange for someone to drive you home.
