Patient Instructions
  1. 1Patients with out of province health cards will be required to make a payment to Middlefield Medical Centre and then will be provided with a reimbursement form and a receipt of payment so that they can be reimbursed from their province of health coverage.
  2. 2Travelers with private insurance or uninsured patients will be charged a flat fee of $75 at registration along with any additional fees as required for procedures. A receipt of payment will be provided.
  3. 3Completion of a travelers insurance form is a separate charge above and beyond the above visit fee.
  4. 4For WSIB visits, please bring completed WSIB form.
  5. 5All patients are expected to come back to Middlefield Medical Centre for follow-up on test results as appropriate.
  6. 6Test results of any kind cannot be given over the phone. The patient must review the results with the doctor in person.
